What is Inscrybe™ e-CMN?
Inscrybe e-CMN is an automated, electronic version of the same paper form used in physician’s offices today. Because it is automated and paperless, Inscrybe e-CMN is more time efficient and cost effective for your practice, freeing up your staff to focus on the issues that are most important to you.

How does it work?
After American HomePatient receives your referral, you will be notified via e-mail that an Inscrybe e-CMN is waiting to be completed—then, it’s as easy as 1 … 2 … 3!

  1. Access the Inscrybe e-CMN server via a secure Internet site
  2. Complete the Inscrybe e-CMN
  3. Electronically sign the Inscrybe e-CMN

Automatic processing takes place the moment the physician completes the electronic signature process.

What is required?
To make the Inscrybe e-CMN work for your practice, you will need:

  1. Access to the Internet (the faster the Internet connection speed, the better)
  2. At least a 200-megahertz PC with the following software installed:
    • Internet Explorer Version 5.5 (upgradeable, generally at no charge)
    • Adobe® Acrobat® Reader® 5.0 (upgrades are available free at www.adobe.com)
  3. Password-protected electronic signature verification for the physician—registration is available online
